Camp and Event Cancellation Form

Day Camp Application Form

Day Camp Final Report Form

Delegate Submission Form
Community/area leaders should submit this online form to let council staff know that their community/area has elected a delegate and delegate alternate.
Family Cookie Responsibility Form
Each participating Girl Scout family must fully read and acknowledge understanding of the adult/family responsibilities and cookie sale procedures prior to participating in the Girl Scout Cookie Program, including cookie booths, before Girl Scouts begin selling cookies.

Girl Scout Bank Account Record Form

Supplemental Insurance Request Form
Use this online form if you are coordinating an event in which non-members are attending or you are planning an activity that will last longer than two consecutive nights.
Travel Request Form
Trip leaders should submit this form prior to the bulk of the trip planning (to ensure approval), but no later than six months prior to departure for trips lasting three nights or more, trips involving air travel, and trips involving three or more troops or more than one community/area.
